Antidepressants Boost GI Bleeding Risk
(HealthDay News) -- Antidepressant drugs called selective serotonin reuptake inhibitors (SSRIs), which include Celexa, Paxil, Prozac and Zoloft, can double the risk of gastrointestinal bleeding, and the threat is more than six times higher if patients take aspirin and similar pain medications at the same time as SSRIs, a new study finds."Clinicians who prescribe these medications should be aware of the potential risk and may need to consider alternatives," senior researcher Dr. Sonal Singh, assistant professor of internal medicine at Wake Forest University School of Medicine, in Winston-Salem, N.C., said in a prepared statement.
His team published the findings in the journal Alimentary Pharmacology & Therapeutics.
In addition to depression, SSRIs are also used to treat panic disorder and obsessive-compulsive disorder. There's growing evidence that SSRIs may be associated with upper gastrointestinal (GI) bleeding.
Singh and colleagues analyzed data from four studies involving 153,000 patients. They found that those taking SSRIs were nearly twice as likely to develop upper GI bleeding as people who weren't taking the drugs.
When patients taking SSRIs also took nonsteroidal anti-inflammatory drugs (NSAIDs) -- such as aspirin, Aleve and Celebrex -- they were six times more likely to develop upper GI bleeding than people who weren't taking either type of drug.
The combined use of SSRIs and NSAIDs may have a "synergistic effect" that greatly increases the risk of upper GI bleeding beyond the risk posed by each kind of drug alone, the study authors suggested.
"While the risk to an individual may increase by only a small amount, the impact to the general population is likely to be substantial because of the large numbers of people who use these drugs," Singh said.
He and his colleagues estimated that one of every 411 patients over age 50 taking an SSRI, and one out of 82 taking both an SSRI and a NSAID, is likely to develop upper GI bleeding requiring hospitalization.

Stirrups-Free Pap Smear May Be a Welcome Option
New research shows that Pap results are just as accurate when the screen is performed with the patient keeping her feet on the examining table.
There was a real bonus in terms of comfort, too.
"There's about a 50 percent reduction in physical discomfort if women did not use the stirrups," said lead researcher Dr. Dean Seehusen, a family physician at the Eisenhower Army Medical Center at Fort Gordon, in Augusta, Ga. Women also said they felt psychologically less vulnerable, he said.
Seehusen, who published his findings recently in the British Medical Journal, remains hopeful that the practice of giving women a choice -- stirrups or no stirrups -- will catch on.
It could even boost women's health by inspiring them to have the exam more regularly, he added.
Seehusen said he had long suspected that one reason some women avoid potentially lifesaving Pap smears, and the accompanying pelvic exam, is due to the anxiety and discomfort of the stirrups position.
It can raise real anxiety in some women, he said, because "when your feet are in the stirrups you cannot easily get out." Women with mobility problems can also have an especially difficult time, he said. On top of those issues, many stirrups are cold to the touch, as well.
The study involved 197 women, ages 18 and up, who had come to the medical clinic for their annual pelvic exams. They were assigned to undergo the Pap test either in the stirrups or not.
After the tests, the women answered questions about their physical comfort, as well as their psychological sense of vulnerability and loss of control.
The result: The quality and accuracy of the Pap smears were similar, regardless of whether stirrups were used or not.
Seehusen's advice: "If a woman thinks she wants to try this method, she should ask her provider," he said.
Another women's health expert agreed.
"If a doctor can do without using the stirrups and you are more comfortable, by all means ask," said Dr. Celeste Robb-Nicholson, editor-in-chief of the Harvard Women's Health Watch and assistant professor of medicine at Harvard Medical School, Boston.
"If this alternative is offered," she said, "it could result in higher screening rates" for pap smears.
But your doctor may still prefer to do some exams with the stirrups, for better stability, she cautioned. While a simple Pap test may be no problem to perform when a woman's legs are not in the stirrups, more complicated procedures -- such as getting an endometrial biopsy -- might be better conducted with a woman's legs stabilized by the stirrups, Robb-Nicholson explained.
And some doctors may still prefer to do the Pap smear while a woman's feet are in stirrups.
But for those women who are uncomfortable in the stirrups, "It is reasonable to ask" to skip them, Robb-Nicholson said.

FDA OKs Return of Silicone Breast Implants
The FDA gave approval to two companies, Allergan Inc., of Irvine Calif., and Mentor Corp, of Santa Barbara, Calif., to market the implants to all women aged 22 and older. Until now, women have only been allowed access to the implants as part of research studies.
"In 1992 the FDA required manufacturers to stop selling silicone gel-filled breast implants because of lack of data to support marketing approval," Dr. Daniel Schultz, director of the FDA's Center for Devices and Radiological Health, said during a late afternoon news conference.
"What is different today is the amount of clinical and non-clinical data now available on these devices," Schultz said. Based on this new data, the FDA believes the implants are safe and effective, he added.
Allergan and Mentor have gathered the information the FDA required, Schultz said. "FDA's decision was based on a thorough review of the companies' core studies," he said. In addition, the FDA inspected their manufacturing facilities, and "these implants will be manufactured according to FDA's quality system regulations," he added.
To ensure that the products continue to be safe, the FDA has required the manufacturers to continue studies for 10 years, and to have the risks clearly stated on the products' labeling, Schultz said.
"Women should know that breast implants are not lifetime devices," Schultz said. "A woman will likely need additional surgeries on her breasts, at least once over her lifetime."
In reaching the decision, the FDA considered the risks and benefits of silicone implants," Dr. Donna Bea-Tillman, FDA's director of the Office of Device Evaluation, said during the news conference.
"Although there were a number of complications reported in the core studies, including breast hardening, breast pain, breast implant rupture, and the need for additional surgery, most of the women in the core studies reported being satisfied with their results," she said.
In addition, the FDA looked at a report by the Institute of Medicine, which found no cases of connective tissue disease or cancer associated with silicon implants, Bea-Tillman said.
The watchdog group Public Citizen has opposed the use of silicone gel breast implants since the fall of 1988. At that time, they petitioned the FDA to ban them after receiving numerous documents from FDA scientists concerned about their safety.
"The moratorium has been on for 14 years, and there is a lot of pressure from various places, from companies and plastic surgeons, to approve them," Public Citizen's Dr. Sidney Wolfe said. "It doesn't make any sense to approve them now."
Wolfe noted that saline implants are safer and a viable alternative to silicone.
"Saline implants have been approved for a long time," Wolfe said. "We never opposed the approval of those. When a saline implant ruptures, salt water, which is what we are made of, leaks out. When silicone implants rupture, a third of a quart of sticky, highly reactive, silicone gel goes out. It's just a black -and-white difference in terms of the dangers."
According to Public Citizen, the dangers of silicone implants, such as high rates of rupture, the need for repeat surgery and clear evidence of lymph node infiltration and damage by leaked silicone, were known when the devices were approved.
"Silicone gel breast implants are the most defective medical device ever approved by the FDA. The approval makes a mockery of the legal standard that requires 'reasonable assurance of safety,' " Public Citizen said in a prepared statement.
In addition, the group thinks this latest FDA action is a "reminder of the double standard for women vs. men that the FDA has not approved silicone gel testicular implants because of the inadequacy of clinical trials on these devices. Saline breast implants and testicular implants have been approved by the FDA."
Another critic of the FDA's decision sees the approval as the FDA giving in to pressure from corporations.
"This appears to be yet another example of the FDA dismissing scientific evidence in order to appease corporate interests," Rep. Rosa DeLauro, D-Conn., and the top Democrat on the House Appropriations subcommittee in charge of the FDA budget, said in a statement.

A pH Miracle
About 2 years ago I lost a niece to suicide, after this tremendous shock I began to experience depression like I had never experienced before and over a period of time got to the point where not only did I not have any energy what so ever but I had no drive.
I would sleep for up to 12 hours some days and feel like I was coming out of a general anaesthetic for hours after waking up.
I also got to the point where I had to have an afternoon sleep to feel that I could cope with the 2nd shift of the day (the kids), after school.
I also was finding I had little if any patience left for my husband and kids. Around this same period of time I began to experience quite severe attacks of acid just below the sternum.
I went to the doctors reluctantly as I am not a big fan of allopathic medicine.
I had a scan done to see if I had gallstones etc and the deluxe blood test of them all. The scan came back clear, and the tests came back looking really good. I was told there was nothing wrong with me.
I had great health according to the tests.At this point I realised that I needed to look elsewhere but to be honest I had tried lots of alternatives prior to the doctor, which was the last resort at that point.
I then listened to a CD of Tony Robbins about a green drink and how he regained his energy through drinking this 'green stuff'. That was the beginning of my journey back to the house of health and things began to really improve for me on all levels.
I bought Dr. Young's book Sick & Tired? and for the first time ever found amazing common sense within the pages.
I could not believe in all the study of natural medicine etc that I had done I had never heard of any of this before. Within about 3 weeks of drinking the greens I began to regain my strength and for the first time in 2 years felt there really was a light at the end of the tunnel.
I no longer needed to have that afternoon nap I had needed before, I found my patience levels increased dramatically which means so much to me as I was able to enjoy my kids again. As my energy increased more and more and the terrible acid pain left me once and for all I realised that more people needed to know about this approach.

New Approach Could Speed Creation of Flu Vaccines
The technique, called reverse genetics, uses cultured kidney cells from monkeys to cultivate the proteins needed for the vaccine. The old system, which the study authors described as "cumbersome," grows the flu virus in eggs.
"It's the difference between a Cuisinart and chopping your own celery," said Dr. Donald J. Kennedy, professor of internal medicine and infectious diseases at St. Louis University. "It's today's computers versus 20 years ago. Will they both compute? Will one take forever and be huge? Yes."
Kennedy was not involved in the study, which appears in this week's issue of the Proceedings of the National Academy of Sciences.
The senior author of the study is Dr. Yoshihiro Kawaoka, a professor of virology at the University of Wisconsin-Madison School of Veterinary Medicine.
Many people believe the existing system for making flu vaccine is impossibly antiquated. That system grows the flu virus in fertilized chicken eggs. Technicians then take the virus from the eggs and make it inactive. The inactivated virus forms the basis of the flu shots millions of people receive each year.
"This is a process that takes quite some time and the yield of protein is not great," Kennedy explained.
Antigens destined for inclusion in the vaccine are selected around January and the vaccine is not ready until about August, in time for each approaching flu season.
Because the circulating regular flu virus changes every year, a new vaccine needs to be made every year. "Sometimes we get a very good match and the vaccine is very good and sometimes it's a less-than-ideal match," Kennedy said.
With the new reverse genetics method, the necessary proteins or antigens from the flu virus can be produced in tissue cell cultures.
"It's much more rapid, much more efficient," Kennedy said. "Instead of the old cumbersome way with a low yield and more expense, this technology can generate large amounts of these proteins much easier."
He likened the advance to one that took place with the hepatitis B vaccine. When that vaccine first came out in the mid 1970s, researchers had to find infected individuals, collect their blood, purify it and then make the vaccine.
In the 1980s, scientists discovered a way to insert the genetic material for the hepatitis B proteins into yeast cells and grow it as if they were making beer.
The new method for flu vaccine represents a similar quantum leap, Kennedy said.
The production system for influenza vaccine has lately become a much bigger issue on national agendas.
For one thing, it is vital to protect people against seasonal human influenza, which claims 36,000 or more lives each year in the United States alone.
Last year, supplies to the United States were cut in half when the Chiron Corp.'s production plant outside Liverpool, England, experienced contamination problems.
But the issue of vaccine production has taken on more urgency in recent months because of outbreaks of avian flu that have now spread beyond Southeast Asia. Although that virus has not yet acquired the ability to spread quickly from human to human, some 65 people have died, most often after exposure to poultry or other infected birds.
Experts worry that the avian virus, H5N1, could still gather the ability to skip easily from human to human, resulting in a pandemic that could claim millions, rather than thousands, of lives.
"If we get a new pandemic strain, the vaccine will be unmatched," Kennedy said. "That's the catastrophe. If there's a big shift, the flu shot won't be good for the new flu strain."
The new system might circumvent this problem. "Based on this technology, when somebody decides what flu antigen we need in a vaccine, it would be able to be produced much more rapidly and in higher quantities," Kennedy said.
People involved in the traditional method objected to the term "cumbersome," but acknowledged that reverse genetics was probably the only hope for a vaccine against the avian flu.
"It's more important for H5N1 because they had to get rid of the virulence," said Doris Bucher, associate professor of microbiology and immunology at New York Medical College in Valhalla, N.Y. By that, she meant the researchers would be able to create a bird flu vaccine using certain genetic material that would pose less of a health risk to people. In this case, reverse genetics might be the "only way," she added.
Bucher's lab produces the starting strain used by flu vaccine manufacturers worldwide. This year, it produced the H3N2 component of the vaccine, the only lab in the world to do so.
Bucher denied that the existing approach was outdated. "They [the designers of the new vaccine method] have simplified the approach," Bucher acknowledged. "But our approach is Darwinian while theirs is more Intelligent Design. They have one gene, we select the best of 64."
Yet the new technology could soon be ready for widespread use, raising the possibility that it would supplant the old system.
"This is using newer biologic technology in ways that it hasn't been utilized before, and obviously there are going to be discussions," Kennedy said. "Just like the car is safer today than it was 25 years ago, this is a better system. This is another biotech advancement in terms of producing the proteins that are necessary for the vaccine. It won't change the whole concept of the vaccination, but it will be an alternative method that would be faster and less expensive."

Enzyme May Aid People With Celiac Disease
But that dietary restriction may soon come to an end. According to researchers in the Netherlands, an enzyme originally developed for commercial food processing -- AN-PEP -- may be able to break down gluten in the stomach before it reaches the small intestine. That's important, because the small intestine is where celiac disease originates.
"There is a realistic chance that this enzyme can be used to develop an alternative treatment for celiac disease," said the study's lead author, Frits Konig, professor of immunology at Leiden University Medical Center.
"It is very difficult to predict if such an enzyme-based treatment would actually be preferred over a gluten-free diet, but at the very least, it would provide patients with the option to occasionally follow a normal diet -- for example, when going out or during other social occasions, times when a gluten-free diet is certainly bothersome," he said.
Konig and his colleagues reported their findings in the online journal American Journal of Physiology - Gastrointestinal and Liver Physiology.
Celiac disease affects about 1 percent of people in the United States and Europe, according to Konig. It's an autoimmune disease that occurs when the body mistakenly sees gluten molecules as foreign invaders that need to be destroyed. Along with destroying gluten molecules, parts of the small intestine are destroyed during the immune system response. Eventually, it becomes difficult for the small intestine to properly absorb nutrients from food, which can cause anemia and vitamin deficiencies.
Currently, the only effective treatment is to avoid gluten altogether. While this may not sound difficult, Konig pointed out how hard it really is to do. "Gluten is a common protein found in wheat and related cereals like barley and rye. This means many common food products, like bread, breakfast cereals, pasta, cookies, etc., are forbidden," Konig said. "Moreover, gluten is often used as an additive in the food industry, so, many products that are not normally associated with wheat may contain gluten," he added.
The Netherlands researchers aren't the only ones studying enzymes as a treatment for celiac disease. Earlier this week, researchers at Stanford University announced they had also discovered an enzyme -- EP-B2 -- that can help prevent the inflammatory immune system response that occurs when gluten is consumed.
Both the enzymes used in the Dutch study and the California study have only been tested in cultures, not in humans. There is no animal model for celiac disease, so treatments can't be studied in animals, either. Konig said that, in the acidic environment of the stomach, he believes the enzyme used in the Stanford research wouldn't survive. The AN-PEP enzyme, however, should work well in the stomach, he said.
"AN-PEP is very efficient and completely breaks down gluten proteins into fragments that are so small that they can no longer cause inflammation in the intestine," Konig said. "This we have also demonstrated by using white blood cells from patients that respond to gluten and cause inflammation in the intestine. These cells no longer respond to gluten when it has been treated with the AN-PEP enzyme," Konig added.
AN-PEP is derived from Aspergillus niger, a common fungus, according to Konig. Because Aspergillus niger is already used in commercial food processing, he said he doesn't expect there to be side effects from the treatment.
Konig said the next step is to conduct human trials.
Dr. Roshini Rajapaksa, a gastroenterologist at New York University Medical Center, is enthusiastic about the Dutch findings. "This could be a huge advancement in terms of treating a very common disease, and it will help a lot of people who are suffering or are dramatically changing their lifestyle," she said.
Rajapaksa had a word of caution, however. "Right now, we're dealing with chemicals in test tubes, so we don't know about any potential side effects. All sorts of things occur in the human body, though it doesn't look like there's any reason this shouldn't work," she said.

In the U.S.A. there presently is a struggle going on between whether the drug company lovers can force the rest of us to only buy drugs and use nothing else. Some arrogant authoritarian egos will not dare face that the whole of their medical schooling, careers, and lives might have been mostly in pursuit of error divorced from Nature. They will fight us and use their stooges to force their methods upon us, whether we like it or approve of it or not. What they do not see is that the majority of the people are not about to give up their fundamental rights to chose, no matter how much the nightly news tries to scare them.
Treaties and the U.S. courts have decreed that: Once a doctor is licensed, he or she acquires the substantial property rights included in that license. These property rights encompass the right of liberty to use whatever means and methods their professional judgments deem necessary to help their patients the best way they know how. They can also do so without having any unreasonable interference by the state.
To be certain the licensed professionals are not losing their inalienable liberty rights through mere lower level administrative decree and agency fiat, and at the same time to to send a clear message to state regulatory agencies that they should never be unduly influenced by the seductive voices of any one momentarily popular industry, many states have enacted medical statutes generally known as 'Alternative Medicine Practice Acts.'
These laws are designed to help any healers who chose to follow the majority's path of 'licensing' so they can do the best healing they know how any way they can and never be closed to innovation. Of course the patient must always be fully informed, and proper records kept.
Even though the laws may already be on the local books, the agency secretaries and authorities may give you trick answers to throw you off the scent when you call them. You may be told something like: "We have no laws legalizing alternative medicine." They know full well that you are not a Boston lawyer and might not realize what they just said was just bureaucratic doublespeak for: "We are not in the business of approving or disproving any specific therapy [including ozone], that is the job of each individual doctor to do for themselves. We only administer 'freedom to practice' statutes. We did just pass (or update) one of them." Always read the actual statute.
